JOB DESCRIPTION:   Clinical Pharmacist Reply to: dcosta@comforce.com The clinical pharmacist will oversee the daily operations interface with the PBM including the coverage determination process, claims, and formulary maintenance. In addition, the clinical pharmacist will review appeals and grievances. This role will require the clinical pharmacist to mange the P & T new drug therapy review process. The pharmacist will also participate in Geriatric Health Management as a member of the Interdisciplinary Team and perform medication assessments for Medication Therapy Management participants. In addition, the clinical pharmacist will be responsible to take the lead on new department initiatives such as a generic conversion, National Provider Identification and E-prescribing initiatives. The pharmacist will have daily interaction with physicians, nurses, employees, PBM representatives and will supervise pharmacy technicians. 1. Prescription claims will process according to benefit guidelines in a timely manner with less than 0.1% error rate. 2. Ensure that new drugs are reviewed by the Pharmacy &Therapeutics Committee within 90-180 days of coming to market. 3. Coverage determinations, grievances, appeals and exceptions will be processed to meet CMS timeframes. 4. Geriatric Health Management and Medication Therapy Management programs will include pharmacist input on appropriate utilization of drug therapies and include drug utilization review. 5. Comprehension of Part D rules and regulations so as to serve as a resource for the company on the Medicare Part D program. 6. Required Experience: Pharmacy Licensed in California required Excellent speaking and writing skills required Pharm.D. Degree with a residency in Ambulatory, Clinical or Geriatric pharmacy preferred Managed care experience at a PBM, health plan or medical group preferred Medicare Part D experience preferred
